The following forms may be useful to review before your blood donation. All blood donors are required to read "What You Should Know About Your Blood Donation" and "Whole Blood Donor Qualification Information." All first-time donors with the South Bend Medical Foundation will be asked to fill out the "Donor Registration Form." You may print out and fill in this form and bring it with you to your donation appointment. The "Parental Consent Form" must be completed for all volunteer blood donors that are age 16.
